Urinary-Trypsin Inhibitor (UTI) is a glucoprotein proteinase inhibitor that effectively inhibits the activity of various pancreatic enzymes such as trypsin, chymotrypsin, lactate, lipase, and hyaluronidase. It is used to treat conditions such as acute pancreatitis, chronic recurrent pancreatitis, and hemorrhagic, traumatic, and endotoxic shocks. UTI belongs to the Kunitz-type protease inhibitors superfamily and plays a crucial role in many physiological and pathological processes. It is derived from a precursor protein that is processed into distinct functioning proteins. Ulinastatin, one of these functioning proteins, exhibits a strong inhibitory effect on various proteases, as well as sugar and fat hydrolases. It is known to be an early marker of renal tubular involvement and possesses radical scavenging activity. Bikunin, another protein derived from UTI, localizes to the cell membrane. Both free uristatin and bikunin are readily excreted in urine and primarily bound to heavy chains, forming the proinhibitor form in plasma. UTI has a molecular weight of approximately 20kDa and 40kDa as determined by SDS-PAGE analysis.
