REG protein has been found to be stimulated during the regeneration of pancreatic islets, leading to the identification of several Reg-related proteins in humans and other animals. In humans, four REG family genes have been isolated, known as REG 1 alpha, REG 1 beta, REG-related sequence (RS), and HIP/PAP. These Reg-related proteins share a similar structure and function, acting as growth factors for pancreatic beta cells. Consequently, the administration of Reg protein is being explored as a potential therapeutic approach for diabetes mellitus. The amino acid sequence of human REG protein is highly similar to that of rat Reg protein, with a 68% homology. Reg I is mainly expressed in pancreatic beta and acinoductular cells, as well as gastric fundic enterochromaffin-like (ECL) cells. Its production in ECL cells is stimulated by gastrin and the proinflammatory cytokine, cytokine-induced neutrophil chemoattractant (CINC)-2Beta. Chronic hypergastrinemia and Helicobacter pylori infection can also increase Reg production in the gastric mucosa through elevated plasma gastrin concentration and proinflammatory cytokine production. Significantly higher serum concentrations of reg-protein are observed in patients with various pancreatic diseases, acute pancreatitis, chronic relapsing pancreatitis, liver cirrhosis, choledocholithiasis, and various cancers of the digestive system compared to normal controls.
