The C3c component is important in both complement activation pathways. It is cleaved by specific proteolytic systems to form C3 convertase, which releases C3a and the C3b fragment. C3 levels can be low due to decreased synthesis or consumption, while high levels are associated with acute or chronic inflammation, hepatic cholestasis, and late-stage pregnancy. Unwanted complement activation causes tissue damage in various pathological conditions and immune complex diseases. Compstatin is an effective inhibitor of C3 activation, blocking a crucial step in the complement cascade. The specific binding site and mode of action of compstatin on C3 are unknown. The crystal structure of compstatin in complex with C3c shows that it binds to the 4th and 5th domains of the macroglobulin (MG) region. This binding site is part of the structurally stable MG-ring formed by domains MG1-6 and is not near any other known C3 binding site. Compstatin does not affect the conformation of C3c, but it undergoes a significant conformational change upon binding.
